📝 Instructions to Fill Out the Form

  • 🔡 Do not use BLOCK letters when filling out the form.
  • ❗ Fields marked with a red asterisk (*) are mandatory and must be completed.
  • 🖼️ Please upload your latest passport-size photograph in the designated field.
  • 📄 The "School proof certificate" field requires you to upload a self attested scanned copy of your admit card, a board certificate, or a certificate issued by the Headmaster of the school.
  • 💰Payment Details
    • If you are making a payment by Cheque, please write the cheque number in the "Notes" section of the form.
    • If you are making a payment via UPI, please include the Transaction ID in the "Notes" section.

If you have already submitted a form(online/offline) please do not fill out the form again
